a lot of knee pain is caused by the alignment of the hip, SI joint, and ultimately the low back.

hi thanks for joining me uh what we're
about to see is
um a case of knee pain that's actually
being caused by the low back and SI
joint and what's remarkable about this
is that
the patient came in a week ago with pain
only in the gastroc muscles behind the
knee and now she's having pain there and
also in the center of the knee joint she
describes the knee feeling painful and
twisted when she squats down in other
words to When the knee is all the way in
flexion and then when she tries to stand
up from a squat
so that's flexion and extension of the
knee
the muscles in the back of the knee are
spasming as a result of the whole chain
from her lower back and her SI joint and
her hip all the way down starting to
become tight in response to a new
technique that she's studying called
gram technique which involves a lot of
extension through the spine and a lot of
extension through the hip and her body
has yet to really get used to that and
to learn to relax in into and through
that technique but once she gets a
little bit of
um
soft tissue release some foam rolling
some stretching in place her and and
studies gram technique a little a little
more further
she should start to settle in and it
won't be a problem I noticed that her
low back is actually tighter this week
than it was last week and her SI joints
are tighter this week than they were
last week and again the reason for that
is the new technique and she's not quite
she's not quite stretching and rolling
quite enough yet but that's just a
learning curve that her body has to get
over and once she does that she'll be
fine the pain that she's having in the
center of the knee which can be quite
scary
I believe is being caused by the tibia
and the femur being rotated against each
other that's that's one thing that
happens with the knee joint it doesn't
just bend and straighten it also twists
slightly and this kind of twisting or
shearing force can cause quite a lot of
discomfort if you deeply Bend and then
try to extreme to try to extend or
straighten the knee from a deep knee
Bend
so the strategy that today has to be a
two-sided treatment I'll be needling her
face down to loosen the gastrox the
hamstring muscles the popliteius also
doing some deep needling into the back
of the knee joint in order to encourage
the body to open the circulation there
and irrigate the knee joint then I'm
going to turn her face up and needle the
front of the knee joint to help the
femur and the tibia to line up and I'm
going to follow that up finish the
treatment off with some gentle flexion
and extension of the knee to encourage
those bones to line up and go back to
Optimal
okay so this is after all the needling
and after the passive range of motion
exercises she got off the table and did
comment that her knee felt much more
calm she was able to squat and stand up
with very little discomfort she's going
to be a little sore for a few days from
all the motor point and trigger point
needling that's a normal byproduct of of
acupuncture sessions and that's just
fine and what I expect is that a lot of
the pattern of tension and pain will
start to recede and what she'll be left
with is a little bit of discomfort
through the gastroc muscles in the back
like she had a week ago and maybe a
little stiffness or some Sensation that
things are not moving quite quite well
enough in her back and her site joints
so when she comes back I'm going to
treat her only face down as long as her
knee joint pain has calmed down if
that's resolved I'm just going to treat
her face down do some cupping and some
acupuncture and some passive range of
motion to open up and realign the
tissues in her back her SI joint pelvis
hip on down through the knee
